Experts welcome more choice and gradual price stabilization, but see weight-loss efficacy, long-term data and oral obesity drugs that could arrive next year as the variables.
Korea’s obesity drug market reached 819.5 billion won last year, more than tripling in a year, and Wegovy and Mounjaro hold about 86% of it. Now Hanmi Pharmaceutical’s ‘Ephe’, Korea’s first homegrown GLP-1 obesity drug, is stepping in. If it wins approval from the Ministry of Food and Drug Safety (MFDS) in October, launch preparations will start right away. So what does Ephe have in common with the existing drugs, and how does it differ? We line up efficacy, side effects and price side by side, and review what experts expect. ① What kind of drug is Ephe?
GLP-1 is a hormone released from the gut after a meal. It acts on the appetite center of the brain to increase fullness and reduce how much you eat. Wegovy, Mounjaro and Ephe are all once-weekly injections built on this principle. Ephe (generic name efpeglenatide) was developed in-house by Hanmi Pharmaceutical and uses the company’s long-acting platform technology, ‘LAPSCOVERY’, which keeps the drug active longer. Like Wegovy, it is a single-action drug that targets only the GLP-1 receptor, whereas Mounjaro is a dual agonist that acts on both the GIP and GLP-1 receptors.
Hanmi filed for marketing approval in December last year, and the drug has been designated for the MFDS’s fast-track review program for global innovative products (GIFT). The company applied for five strengths, 2, 4, 6, 8 and 10 mg, in 0.5 mL single-use auto-injectors, for adults with obesity who do not have diabetes. Starting doses and dose-escalation intervals will be set in the product label after approval. The plan is to launch within about a month of an October approval, and the official target is a launch within this year. Production will be handled at Hanmi’s Pyeongtaek Bio Plant.
② Efficacy — how much weight comes off?
Ephe’s Phase 3 trial is a randomized, double-blind, placebo-controlled study of 448 Korean adults with obesity and no diabetes. The full study is designed to run 64 weeks, and what has been disclosed so far is the interim result at week 40. Among the 446 patients in the efficacy analysis, body weight in the Ephe group fell by an average of 9.75%, versus 0.95% in the placebo group. Some 79.42% of patients lost at least 5% of their body weight, 49.46% lost at least 10%, and 19.86% lost at least 15%. Among women with a body mass index (BMI) below 30, the average reduction was 12.20%, the highest among the subgroups analyzed.
For comparison, in the STEP 1 trial of 1,961 adults, Wegovy produced an average reduction of 14.9% after 68 weeks, with 86.4% of patients losing at least 5%, 69.1% at least 10% and 50.5% at least 15%. In the SURMOUNT-1 trial of 2,539 people, Mounjaro’s average reduction after 72 weeks was 15.0% at 5 mg, 19.5% at 10 mg and 20.9% at 15 mg; in the 15 mg group, 70.6% lost at least 15% and 56.7% lost at least 20%.
Still, it is hard to line these numbers up and declare a winner. The trials differ in length, population and design, and Ephe’s data are still interim. Kiwoom Securities judged that, once the placebo effect is removed, Ephe’s -8.13% (40 weeks) and Wegovy’s -8.5% (a 44-week trial in Asian participants) look similar in an indirect comparison. Professor Kim Kyung-gon of Gachon University Gil Medical Center, on the other hand, said that in terms of ‘weight-loss target’, Ephe is comparable not to Wegovy or Mounjaro but to the existing oral combination obesity drug Qsymia, which produced an average loss of 9.8 to 10.9% over 56 weeks.
③ Side effects — what to watch for
For all three drugs, the most common side effects are digestive symptoms such as nausea, vomiting, diarrhea and constipation. The drugs slow the emptying of the stomach and affect appetite and fullness signals, so these symptoms are common early in treatment or when the dose is raised, and mostly ease over time. In Ephe’s Korean Phase 3 trial (40 weeks), nausea was reported in 16.72% (placebo 5.37%), vomiting in 11.71% (placebo 2.01%) and diarrhea in 17.73% (placebo 4.7%). Hanmi says the overall rate of digestive side effects was 50%, lower than in the Wegovy trial.
In the Wegovy STEP 1 trial, nausea was 44.10% (placebo 17.40%), vomiting 24.58% (placebo 6.56%), diarrhea 31.47% (placebo 15.88%) and constipation 23.35% (placebo 9.47%). For Mounjaro, US FDA approval documents report nausea of 25 to 29% (placebo 8%) and vomiting of 8 to 13% (placebo 2%). In the 72-week SURMOUNT-5 trial, which compared Wegovy and Mounjaro head to head, treatment discontinuation was 6.1% for Mounjaro and 8.0% for Wegovy, and discontinuation due to digestive symptoms was 2.7% and 5.6%, respectively. However, the Ephe and Wegovy figures come from different trials, so no verdict on relative safety can be drawn, and data on Ephe’s constipation and other adverse events, as well as its long-term safety, have not yet been fully disclosed.
There are also cautions that apply across the GLP-1 class. Gallstones and cholecystitis were reported at 1.6% (placebo 0.7%) and 0.6% (placebo 0.2%) in Wegovy trials, and at 1.1% (placebo 1.0%) and 0.7% (placebo 0.2%) for Mounjaro. On acute pancreatitis, a large study of about 66,000 people found a slightly higher risk among users of GLP-1 obesity injections, but there is still no clear evidence that the drugs directly damage the pancreas, and some analysts say it may be a side result of rapid weight loss. Mounjaro’s labeling in Korea also warns that severe dehydration from vomiting and diarrhea can lead to acute kidney injury. Item-by-item figures like these have not yet been disclosed in detail for Ephe, so the product label should be checked after approval. The MFDS is moving to designate these GLP-1 obesity drugs as ‘drugs of concern for misuse and abuse’.
④ Price and supply — why price is the battleground
Obesity drugs are not covered by national health insurance, so patients pay the full cost. At pharmacies in Seoul, a month of treatment costs roughly 200,000 to 400,000 won for Wegovy and 300,000 to 700,000 won for Mounjaro. The ex-factory price for four weeks at the starting dose is 216,000 won for Wegovy (0.25 mg) and 278,000 won for Mounjaro (2.5 mg). Hanmi has not disclosed Ephe’s supply price, but the industry is talking about a level of around 100,000 won for four weeks. The rationale is that in-house production can cut manufacturing and distribution costs.
The wild card is how the incumbents respond. Ahead of Mounjaro’s Korean launch, Wegovy cut its starting-dose supply price by about 42%, from 372,000 won to 216,000 won. A price war could break out again once Ephe arrives. On supply, domestic production is seen as a strength. Wegovy and Mounjaro have both run short of some doses since launching in Korea because of strong demand, while Hanmi plans to respond flexibly to changes in demand with its own production system.

⑥ What do experts expect?
First, the view from clinical practice. Professor Kim said it is a positive that a new drug widens prescribing options at a time when high drug prices make it hard for patients who really need treatment to start it. But he expects the launch to have little immediate effect on prices of imported obesity drugs, and said gradual price stabilization can be hoped for over the longer term as more treatments arrive and competition intensifies. A pharmaceutical industry source also said Ephe has differentiators in price and in clinical data from Korean patients, but that Wegovy and Mounjaro have high awareness and prescribing experience, so the key is how far Ephe can change the choices of doctors and patients in actual practice.
The brokerage community is relatively upbeat. Hanmi has given 2027 sales guidance of around 100 billion won for Ephe, and KB Securities analyst Shin Ji-hoon said that given the steep growth of Korea’s obesity drug market, exceeding that guidance is possible. Some in the industry also see price competitiveness as the decisive weapon, and say that the company’s ability to raise utilization of the Pyeongtaek plant and prove mass-production cost savings will be a key yardstick for re-rating its corporate value.
The biggest outside variable is the oral obesity pill. Novo Nordisk’s Wegovy pill (semaglutide) and Eli Lilly’s Foundayo (orforglipron) have already filed for approval with the MFDS, and the industry expects a Korean launch next year. Oral versions are usually cheaper than injections, which could heat up price competition further. It is also worth remembering that market rankings can flip at any time. Mounjaro, a latecomer, overtook Wegovy in Korea, and as of June this year its share of prescriptions had widened to about nine to one.
Hanmi has also laid out a roadmap beyond Ephe. Follow-on candidates include HM17321, a muscle-gain obesity treatment, and HM15275, a triple agonist that acts on the GLP-1, GIP and glucagon receptors at the same time. HM15275 is in a Phase 2 trial in the United States, expected to finish in the first half of 2027, with a commercialization target of 2030. Analysts say that securing market share and contact with physicians through Ephe would let Hanmi lean on its existing sales and marketing base when it brings the next generation to market.
Good to Know
Ephe is not yet approved. The timing of approval, launch date and price are not confirmed, and a price around 100,000 won is only an industry expectation.

It is a drug trying to pry open a gap in the market with price, stable supply, data from Korean patients and comparatively lower digestive side effects. Its success will depend on the final 64-week results, its reception in real-world prescribing, and the final supply price. Add the incumbents’ price responses and the arrival of oral obesity drugs, and Korea’s obesity drug market from next year onward is likely to become a far more complicated contest than today. As choices widen, it matters for patients to weigh efficacy, side effects and cost together with their doctors.

증권가는 비교적 긍정적이다. 한미약품은 에페의 2027년 매출 가이던스로 1,000억 원 수준을 제시했고, 신지훈 KB증권 연구원은 국내 비만치료제 시장의 가파른 성장세를 감안하면 가이던스 초과 달성도 가능하다고 분석했다. 업계에서는 승부수가 단연 가격 경쟁력이고, 평택 바이오플랜트의 가동률을 끌어올려 대량생산 원가 절감 능력을 입증하는지가 기업가치 재평가의 핵심 기준이 될 것이라는 시각도 나온다.
가장 큰 외부 변수는 ‘먹는 비만약’이다. 노보 노디스크의 위고비필(세마글루타이드)과 일라이 릴리의 파운다요(오르포글리프론)는 이미 식약처에 품목허가를 신청했고, 업계는 내년 국내 출시가 가능할 것으로 본다. 경구용은 통상 주사제보다 가격이 낮아 가격 경쟁이 한층 뜨거워질 수 있다. 시장 순위가 언제든 뒤집힐 수 있다는 점도 눈여겨볼 대목이다. 후발주자였던 마운자로는 국내에서 위고비를 앞질러, 올해 6월 기준 처방 비중이 약 9대 1까지 벌어졌다.
한미약품은 에페 이후의 로드맵도 제시했다. 근육 증가형 비만치료제 HM17321과 GLP-1·GIP·글루카곤 수용체에 동시에 작용하는 삼중작용제 HM15275가 후속 파이프라인이다. HM15275는 미국 임상 2상을 진행 중이고 2027년 상반기 종료가 예상되며, 상용화 목표 시점은 2030년이다. 에페로 시장 점유율과 의료진 접점을 확보해 두면 차세대 제품을 낼 때 기존 영업·마케팅 기반을 활용할 수 있다는 분석이다.
알아두면 좋은 점
에페는 아직 허가 전이다. 허가 시점과 출시일, 가격은 확정된 것이 아니며 10만 원대 가격도 업계 관측일 뿐이다.
서로 다른 임상시험의 체중 감소율과 부작용 발생률을 그대로 비교해 약의 우열을 판단하면 오해할 수 있다. 에페의 공개 수치는 40주 중간 결과다.
비만치료제는 의사의 진단과 처방이 필요한 전문의약품이다. 복용 여부와 용량은 본인의 건강 상태에 맞춰 의료진과 상의해야 한다.

The US and Japan raised rates back to back, yet semiconductors lifted stocks in all three markets

This week brings the UN General Assembly debate on September 22 and a US–China summit on September 24, while Korean markets are closed on September 24 and 25 for Chuseok.
The biggest story in global markets last week was interest rates. The US Federal Reserve (Fed) raised its benchmark rate on September 16 (US time) for the first time in three years and two months, and two days later the Bank of Japan (BOJ) followed with a hike of its own. Rate hikes are usually treated as bad news for stocks. This time, the outcome was different. The KOSPI won back the 7,000 level on the 21st, and chip-related shares were strong in New York and Tokyo as well. Here is how the Korean, US and Japanese markets each moved, and why, explained simply.
① Why do rate hikes rattle stocks?
Because borrowing costs rise for companies and households, and future profits are valued lower when converted into today’s money. That is why growth stocks, especially tech, are more sensitive to rate hikes. This hike was a potential headwind for markets, too. All 12 voting Fed members backed it, and the median of the ‘dot plot’, which shows each policymaker’s rate forecast as a dot, rose to 4.1%, pointing to one more hike this year. Of the 18 officials who submitted projections, 16 saw another increase at either the October or December meeting. Fed Chair Kevin Warsh explained the decision by saying inflation has stayed above the target for more than five years, while President Trump pushed back, saying rates should be cut sharply. Still, markets had largely priced in the move. Just before the meeting, the probability of a hike stood at 93.5% by the FedWatch measure. Once the result came out, uncertainty faded, and attention shifted to ‘how much more will they raise?’
② Korea — Recovering 7,000 after a four-day slide
The KOSPI closed at 7,033.92 on September 10, then began to slide. Over the next four sessions it fell about 6%, to 6,627.26 on the 15th. Foreign investors were net sellers for six straight sessions, with cumulative net selling of about 11.9 trillion won. On the 16th, ahead of the FOMC decision, dip-buying in chip stocks lifted the index 1.37% to 6,717.97. On the 17th, even after the rate hike was announced, the index slipped just 0.04% to 6,715.41, so the shock was limited.
The mood changed on the 18th. The KOSPI rose 2.66% to 6,894.23, with foreign investors net buying 970.1 billion won and institutions net buying 1.7173 trillion won. Individual investors sold 4.3652 trillion won. Then on the 21st, Samsung Electronics gained 4.98% and the KOSPI rose 1.65% to close at 7,007.72, regaining 7,000 on a closing basis. Fund flows, however, were different that day. Institutions net bought about 1.5 trillion won, while individuals net sold about 3 trillion won and foreign investors were small net sellers. ‘Other corporations’, believed to reflect share buybacks by Samsung Electronics and SK hynix, net bought in the 1.6 trillion won range, which supported the index. While chips rose, battery and auto stocks such as LG Energy Solution (-3.30%) and Hyundai Motor (-1.64%) fell. The KOSDAQ rose 1.11% to close at 836.27.
The Bank of Korea’s benchmark rate stands at 3.00% after back-to-back hikes in July and August, and the gap with the US (upper bound) has widened to 1.00 percentage point. If US rates rise further, that gap could affect foreign fund flows and the won-dollar exchange rate, so it is worth watching.
③ US — Indexes split, only chips smiled
On the 18th (US time), Wall Street ended mixed. The Dow Jones Industrial Average fell 0.18% to 51,682.64, the S&P 500 rose 0.17% to 7,650.50 and the Nasdaq Composite rose 0.39% to 26,522.55. The split came down to Treasury yields and semiconductors. Treasury yields are market-set rates, and when they rise, stocks look relatively less attractive. The 10-year yield moved back above 5%, and the 2-year rose to 4.741%, the highest since July 2024, weighing on sentiment.
On the other hand, the Philadelphia Semiconductor Index jumped 2.78%. Nvidia rose 1.34%, Broadcom 2.97%, AMD 2.70% and Micron 3.92%. For the week as a whole, the Dow fell 1.7% and the S&P 500 fell 0.1%, but the Nasdaq gained 0.7%, showing momentum tilting toward tech. Oil prices falling for a third straight day also gave the market some breathing room.
④ Japan — Highest rates in 31 years, so why did the Nikkei rise?
The Bank of Japan raised its policy rate from 1.0% to 1.25% on the 18th. It is the highest level in 31 years, since 1995, and seven of the nine board members voted in favor. The BOJ explained that high oil prices, a weak yen and rising AI demand are pushing prices up. Governor Kazuo Ueda said, in effect, that the weight has shifted from lifting inflation to containing the risk of prices overshooting the target, and he hinted at further hikes.
Even so, the Nikkei 225 rose 1.38% to close at 65,018.95, its third straight gain. Relief that the hike was as expected, combined with strength in US chip stocks, drove the move. Kioxia Holdings jumped 9.40%, Advantest 5.99% and Tokyo Electron 4.20%, lifting the index. But TOPIX, which covers the broader Tokyo Stock Exchange, slipped 0.07% to 4,091.14, and 64% of Prime market stocks fell. In other words, a handful of chip names created an illusion of broad strength. The yen also failed to gain from the hike and stayed weak. Note that Japanese markets are closed from the 21st through the 23rd for holidays.

Notes for Investors
With the US 10-year yield hovering around 5% and further hikes possible this year, volatility in growth and chip stocks could rise again depending on the direction of rates.
A rising index does not mean the whole market is rising. In Japan, 64% of Prime market stocks fell on the day the Nikkei rose, and on the KOSPI on the 21st battery and auto stocks were weak. The deeper the concentration in chips, the wider the gap between the index and the returns investors actually feel.
The KOSPI’s rise on the 21st was supported by institutions and money estimated to be from share buybacks, while individuals and foreign investors were net sellers. It is worth watching whether that pattern continues.
Overseas risks continue through the Chuseok holiday, including the US–China summit, the UN General Assembly and Micron’s earnings. Daishin Securities calculated that the KOSPI’s average return over the five sessions after the Chuseok holiday over the past 10 years was +0.68%, but there is no guarantee that past statistics will repeat.
In short, the simple formula ‘rate hike = bad for stocks’ did not hold this time. With the US and Japanese hikes concluding within expectations, uncertainty eased, and chip and AI-related stocks filled the gap. But the rally was concentrated in semiconductors in all three markets, and rates could still rise further. Heading into the holiday, look beyond the index level and check which sectors rose and who was buying.

① 금리를 올리면 왜 주가가 흔들릴까
기업과 개인이 돈을 빌리는 비용이 늘어나고, 미래에 벌 이익을 지금 가치로 환산할 때 더 낮게 평가되기 때문이다. 그래서 앞으로의 성장 기대가 큰 기술주일수록 금리 인상에 민감하다. 이번 인상도 시장에는 부담이 될 만한 재료였다. 연준은 투표권을 가진 위원 12명 전원이 찬성했고, 위원들의 금리 전망을 점으로 표시한 점도표의 중간값은 4.1%로 올라 연내 한 차례 추가 인상이 예상된다. 점도표에 의견을 낸 18명 중 16명이 10월이나 12월에 한 번 더 올릴 것으로 봤다. 케빈 워시 연준 의장은 인플레이션이 5년 넘게 목표치를 웃돌고 있다며 인상 배경을 설명했고, 트럼프 대통령은 금리를 크게 낮춰야 한다며 반발했다. 그럼에도 시장은 이미 인상을 상당 부분 예상하고 있었다. 회의 직전 페드워치 기준 인상 확률이 93.5%였기 때문이다. 결과가 나오면서 불확실성이 걷혔고, 시장의 관심은 ‘얼마나 더 올리느냐’로 옮겨갔다.
② 한국 — 나흘 하락을 딛고 7,000선 탈환
코스피는 지난 10일 7,033.92로 마감한 뒤 내림세를 탔다. 이후 나흘 동안 약 6% 떨어져 15일에는 6,627.26까지 밀렸다. 외국인은 6거래일 연속 순매도를 이어갔고, 누적 순매도액은 약 11조 9천억 원에 달했다. 16일에는 FOMC를 앞두고 반도체 저가 매수가 들어와 1.37% 반등한 6,717.97에 마감했다. 17일에는 금리 인상 결과가 나왔는데도 0.04% 내린 6,715.41에 그쳐 충격이 크지 않았다.
분위기가 바뀐 것은 18일이다. 코스피는 2.66% 오른 6,894.23에 마감했고, 외국인이 9,701억 원, 기관이 1조 7,173억 원어치를 순매수했다. 개인은 4조 3,652억 원어치를 팔았다. 그리고 21일, 삼성전자가 4.98% 오르면서 코스피가 1.65% 상승한 7,007.72로 마감해 종가 기준 7,000선을 되찾았다. 다만 이날 수급은 달랐다. 기관이 약 1조 5천억 원을 순매수했지만 개인은 약 3조 원, 외국인은 소폭 순매도했고, 삼성전자·SK하이닉스의 자사주 매입으로 추정되는 기타법인이 1조 6천억 원대를 순매수하며 지수를 받쳤다. 반도체가 오르는 사이 LG에너지솔루션(-3.30%)과 현대차(-1.64%) 등 2차전지·자동차주는 내렸다. 코스닥은 1.11% 오른 836.27에 마감했다.

③ 미국 — 지수는 엇갈리고 반도체만 웃었다

그런데도 닛케이225는 1.38% 오른 65,018.95로 마감해 3거래일 연속 상승했다. 인상이 예상대로였다는 안도감에 미국 반도체 강세가 겹친 결과다. 키옥시아홀딩스가 9.40%, 어드반테스트가 5.99%, 도쿄일렉트론이 4.20% 오르며 지수를 끌어올렸다. 하지만 도쿄증권거래소 전체를 보여주는 토픽스는 0.07% 내린 4,091.14였고, 프라임 시장 상장사의 64%는 주가가 내렸다. 반도체 몇 종목이 지수를 끌어올린 착시가 있었다는 뜻이다. 엔화도 금리 인상에 힘을 받지 못하고 약세를 이어갔다. 참고로 일본 증시는 21일부터 23일까지 연휴로 쉰다.
